New Patient Registration
It is necessary for all new patients wanting to register with the practice to download the initial registration form following the link below, as well as completing our own Broom Leys registration pack. PLEASE NOTE: all registration packs must be handed to a member of the team along with two forms of identification (passport or driving license & a utility bill with name and new address).

Please download and complete BOTH of the above forms and return them to the surgery with your Identification as stated below.
Please note: We request TWO forms of identification alongside your completed registration forms. These are to include:
- Passport or Driving Licence (Photo ID)
- Utility bill or bank statement (proof of address)

From April 2015 all patients, including children, must be allocated a named accoutable GP, who will have overall responsibility for the care and support that a surgery provides to them.
This does not mean that you will only see your named GP, your care will still be provided by the most appropriate member of the clinical team here at Broom Leys Surgery. Having a 'named GP' does not prevent or restrict you from seeing any other doctor or nurse in the practice as you currently do. Continue to book your appointments with the practice in the same way that you do now.
